Disability Living Allowance (DLA) forms

You may be eligible if you experience the following:

Lack of motivation, need for prompting and encouragement to get up in the mornings, to eat, use the toilet, take medication, dress and undress, wash, bathe and look after your appearance.

Concentration so poor that claimant cannot plan a main meal, ie a proper meal, not a sandwich or microwave meal.

Requirement for supervision throughout the day and night as claimant is a danger to themselves or others, e.g. is verbally, physically aggressive, suicidal, etc.

Anxiety and panic attacks outdoors on unfamiliar routes

Mental illness makes the claimant think they have a problem walking or they have severe behavioural problems requiring constant supervision outdoors
