We are a community based Recovery and Support team providing a service for adults aged between 18 - 75 who are experiencing severe and enduring mental health conditions. Our multi-disciplinary team consists of different mental health professionals such as psychiatrists, psychologists, community psychiatric nurses, occupational therapists, and recovery and support workers.
We provide evidence based treatments and support interventions to enable recovery and assist our service users to self-manage their condition.
What is The Recovery Approach?
Recovery is a whole service approach which is respectful, collaborative, and which supports and promotes empowerment of the person experiencing mental illness. Recovery is not necessarily about finding a 'cure' but is about supporting people with mental illness to live a meaningful and satisfying life.
We work closely with social services and the Mental health social work team, GP practices, carers and families. We also work closely with local voluntary and community organisations to support you in your home.
The team provides an intensive support service (known as the FACT - flexible assertive community treatment) when people have periods of greater need and are most unwell.

Our service provides care coordination for people who have severe and long term mental illness. We care and support people who live in London Borough of Sutton.
|How to refer:|
Referrals to our service are made by the Adult Mental Health Assessment Team which is based with us at Jubilee East Health Centre.
We also referral from assessment team, Home Treatment Team, Inpatient Service, Forensic Team and Early intervention Team and others.
