Service offered:
Richmond CMHT is a Community Mental Health Team serving the local and surrounding community. It is a multi-disciplinary team. It works with clients/patients and their families who have a range of severe and enduring mental health disorders. Providing social care assessment and working closely with clients or patients who need intensive support for a limited time to help them recover from episodes of illness and distress. Work is based on the recovery model and there are a range of supporting services that help in delivering the right services at the right time.
Who is the service for?:
We work with adults between the ages of 18-65. Richmond CMHT is the gateway to other specialist mental health services and some clients will be assessed by our services and then transferred to teams that can better support recovery.
How to refer:
Need to have had a valid referral forwarded on the patient's behalf to the service by GP, hospital facility, mental health facility, out of hours facility etc.
