Who we are
The team offers support to people in the borough aged 18 and over who experience mental health problems and have a learning disability. The team also supports the person’s family or carers.

This service is for adults aged 18+ who live in the borough of Wandsworth and who have a learning disability and a mental health problem.

What you can expect
People who use our services first get an assessment of their mental health needs and a plan of the care and support they will receive. They will also will also have one named person who co-ordinates their care and support. This person will be called either a named professional or if your needs are more complex, a care co-ordfinator. This person should be your main point of contact. You can speak to them about how your treatment and support are going.
Referrals are accepted from Healthcare professionals, social workers, support workers and other carers
You will be given an appointment with the service following referral, if the referral is accepted by the team
