Young people aged 11-17 years with complex clinical, social and educational problems such as violent behaviour, drug abuse and school expulsion and who are at high risk of failing into care or the criminal justice system.
Anyone who uses our services first gets an assessment of their mental health needs and a plan of the care and support they will receive. They will also will also have one named person who co-ordinates their care and support. This person will be called either a named professional or, if your needs are more complex, a care co-ordinator. This person should be your main point of contact. You can speak to them about how your treatment and support are going.
The MST Team accepts referrals from Merton, Sutton, Wandsworth, Kingston and Richmond on a spot purchase basis.
Completed MST referral form should be emailed to the MST Team. email@example.com
MST blank referral form can be requested from: firstname.lastname@example.org
One of the MST Supervisors will then arrange to meet the family to do a suitability assessment for MST. following that assessment, the MST Supervisor will provide feedback to the referrer. If the referral meets criteria for MST, referrers will need to get written approval for funding from either the CCG commissioner or the relevant Social Care management team.
Discussion with the MST Supervisor aroung potential referrals are very welcome.
