Who we are

This Community Mental Health Team provides treatment and care to adults in their own homes and in the community.

Who is the service for

This service provides treatment for adults suffering severe and enduring mental illness.

What you can expect

People who use our services receive a plan of the care and support they will receive. They will also will also have one named person who co-ordinates their care and support. This person will be called either a named professional  or,  if your needs are more complex, a care co-ordinator. This person should be your main point of contact and you can speak to them about how your treatment and support are going.

Referrals

Your GP should always be your first point of contact for any mental health related issues.

Your doctor can then refer to the Single Point of Access (SPA) if appropriate. Referrals are usually via letter or efax to the SPA team.
