| Service offered: |
| Patients are assessed and treated for functional illnesses such as depression, anxiety states and schizophrenia. All conditions seen by the adult service are also treated, including organic mental health conditions such as Alzheimers and dementia. The treatment involves general observations, which requires: blood pressure monitoring, temperature, urinalysis as well as height and weight (bmi) calculation. The initial interviews are carried out by a nurse and occupational therapist, who devise a risk assessment and care plan. |

| Ages are specific due to the catchment area. |
| How to get referred: |
| Patients are referred to the Community Mental Health Tem in the first instance. They will then be contacted and given an appointment. Referrals should include reasons why the patient is being referred, their current medication list, a risk assessment and a care plan. |
| Additional information: |
| This service is for short-term treatment only and patients will be discharged once their care is complete. Patients can also be referred from the ward if they are in need of additional support on discharge. |
