Reporting
As soon as you know that you are going to be absent due to illness, you must telephone your department and inform your Practice Placement Educator or other staff they have designated. Other than in exceptional circumstances, you should call prior to the time that you would otherwise start work.
You should state: -
a) How long you expect to be absent (e.g. one day. one week).
b) Whether your absence is due to an injury attributable to your work or a disease contracted at work. This will ensure adherence to the legal requirements and rights of employees in relation to industrial injury.
c) In order to ensure that appropriate cover can be organised, you must keep your Practice Placement Educator informed of your progress, particularly if the date you expect to return changes or if you are visiting your G.P.
d) When you are fit to return to duty, you must inform your Practice Placement Educator or other designated person in advance of your day of work.
Certificates:
a) Up to 3 calendar days - No certificate required
b) 4 - 7 calendar days - a self certificate is required. This can be obtained from your GP.
c) 8 calendar days or more - a medical certificate is required. This is usually Form Med 3 (white), issued by your GP. If you are in hospital. Form Med 10 (yellow) will be issued, these must be sent by post or delivered by hand to your Practice Placement Educator as soon as possible after they are issued. It is your responsibility to ensure that certificates to cover your absence are received as soon as they are due.
