Co-Creating Health


The Co-Creating Health initiative (CCHi) is a national project which enables people with a variety of long-term health conditions take a more active role in their own care. The aim of the CCHi is to encourage and support healthcare professionals and people with long-term conditions to work together in an equal partnership. This is what we mean by 'co-creating health'.

The project comprises three strands:

  1. A Self-Management Programme (SMP) is a free course for people living with depression.  It is co-facilitated by a lay tutor, someone with experience of self-management and depression, and a healthcare professional.
       
  2. An Advanced Development Programme (ADP) providing an opportunity for healthcare professionals to enhance skills in supporting people to manage their long-term conditions.
       
  3. A Service Improvement Project (SIP) which aims to ensure that the health service supports healthcare professionals and service users to use self-management principles.
