How to access your health records

Under the Data Protection Act (1998) you have the right to see or obtain a copy of your health records. An application for access to your health records, or to any part of your health records, may be made to the Information Governance Manager.:

You can do this by writing to or email the Information Governance Manager for an application form and guidance notes to help you in your application.
